WHAT IS STRUCTURED FAMILY CARE?
Structured Family Caregiving provides support, education, and oversight on behalf of waiver members(CLIENT) whose family caregiver lives in the home with the member. The service provides a community-based option of continuous care, support, and professional oversight by promoting a collaborative relationship between the waiver member(CLIENT), the primary caregiver, the professional Health Coach, and the member’s case manager.
Structured Family Caregiving allows family members to care for their own family members, providing financial encouragement for dedicated family caregivers to take care of their loved ones in a safe and familiar environment with love.
(CCSP or SOURCE) Member Eligibility To be eligible
To be eligible to participate in the SFC service, a CCSP or SOURCE member must meet the following qualifications:
- They are an adult at least 18 years of age.
- They have current Waiver Medicaid eligibility or are eligible for Medicaid
- They require at least 5 hours of daily(7 days) personal care services, such as those provided in a nursing home, as determined by their most recent assessment and service plan for CCSP or SOURCE services.
- They live with a family member who provides the hours of care described above. (same home/same address)

Caregiver Eligibility and Responsibilities
To be eligible to participate in the SFC services, the family caregiver of a CCSP or SOURCE member must meet the following qualifications:
- They are at least 18 years of age and can’t have another job working with the SFC program
- They are related to the CCSP or SOURCE member as
- They are NOT the spouse, legal guardian, or conservator of the family member for whom they are providing care. Eligible to provide care: cousin, nephew, niece, son, daughter, aunt, uncle, or related by marriage
- They live in the home with the CCSP or SOURCE member for whom they are providing care. (same home/same address)
- They pass a criminal background check conducted by the SFC agency that verifies they have not been convicted of any of the felonies listed
- They are willing to work with the SFC provider’s health coach
- They are responsible for supporting the member to maximize the highest level of independence possible by providing necessary care and support, including the following:
